| 1 Qualification
requirements of Telecommunication Terminal Testing & Approval Forum member |
| Any communication operator, communication
equipment manufacturer, research and development design organization, approval
organization, approval testing lab and testing equipment manufacturer who have
the corporation qualifications are willing to apply for the member, acknowledge
the forum constitution, undertake the member obligations and pay the member fee
regularly can become the forum member after the approval by the council. |
| 2 The following materials
should be provided by the organizations who apply for joining this forum: |
|
（1） Written application for joining this forum
The candidate
organization need download the
member application form offered
by the
website, fill in it
carefully and then got it stamped
(Note: the application
form should be submitted by the printing copy)
（2） The corporation certification material of the candidate
organization
The candidate
organization should submit the copy of its business license, signed by the
legal person of the
organization and marked with “Identical with the original copy”,
signed with the data of
copy and stamped.
（3） Introduction of the organization
The candidate
organization should submit the detailed introduction of its organization and
got it stamped.
（4） Sign the self-discipline claim of this forum
The telecommunication
equipment manufacturers should download and sign “
enterprise self-discipline claim”
and got it stamped.

（1） After the preparation of the application
materials according to the requirements, post it

District”

（2） The forum secretariat implements initial review after
receiving the application materials.
（3） The member qualification will be granted after it is
discussed and approved by the
council.
（4） The forum secretariat notifies the review results to the
candidate organization within 5
business days.
（5） When the candidate organization has member
qualification, the forum secretariat will
issue the member
certificate.
